Please submit the Skin Consultation Form after submitting your Deposit . All consultation forms for Chemical Peel , MicroNeedling , Laser Hair Removal & other advanced procedures we offer will be sent out via email please lookout for a email after booking.

Please Note : ALL First Time Clients MUST book under ”Ultimate Detox + Skin Consultation Facial” , Returning Clients please book under our Monthly Maintenance Facial unless advanced treatments were previously discussed.
